SYDNEY, NEW SOUTH WALES, AUSTRALIA — BHP says it is reviewing contractor arrangements following two worker deaths at mines it operates, with the company’s chair telling shareholders that the group accepts operational responsibility at both sites.
The comments were made at BHP’s annual general meeting in Sydney on 22 October 2026. The fatalities occurred at the Peak Downs mine in Queensland in July and the Escondida mine in Chile in September. Both incidents pre-date the meeting; the fresh development is BHP’s public description of its safety governance and contractor review.
Board describes review and oversight process
Chair Ross McEwan said BHP was asking hard questions after the fatalities and remained focused on eliminating deaths and serious injuries. Reporting from the meeting says BHP is assessing whether contractor arrangements provide the safety protections expected for employees.
The company also described an internal review structure in which a senior executive outside the affected business leads the investigation, with reports moving through the board’s sustainability committee. These statements describe company governance; they are not final regulator findings on either death.
Contractors included in the same safety outcome
Contractor management is a persistent mining risk because work is often split across operators, specialist firms and changing crews. Site access, competence checks, supervision and critical-control verification need to function as one system. HSE Blog’s contractor safety induction guide explains pre-arrival and site-specific controls, while its 2026 mining-safety guide covers high-consequence operational hazards.
HSE Blog analysis
The significant development is BHP’s acknowledgement that operated joint ventures remain its operational responsibility and that contractor arrangements are under review. The value of that review will depend on whether it produces verified changes to critical controls, accountabilities and assurance across both employees and contractors.
